Managing you Health Care


Living a Healthy Lifestyle

Physicians Medical Group of San Jose and your managed care health plan believe in preventative care. We encourage a healthy life style – diet, exercise, no smoking, etc. In addition, we believe all adults and children should follow recommended preventative guidelines including doctor’s visits and immunizations.

When You Become Ill

When you are not feeling well or if you have an ongoing medical condition, see your Primary Care Physician (PCP). He or she is responsible for coordinating all of your health care needs, including specialist referrals, emergency and urgent services, and hospitalization.

If you become unexpectedly ill and require immediate medical attention, we encourage you to call your Primary Care Physician. Your physician or an on-call physician is available 24 hours a day, 7 days a week. Rather than wait in an emergency room and be cared for by a physician who does not know your medical history, your physician prefers that you call his or her office.

If you are unable to reach your physician and need immediate care for an urgent medical problem, your doctor may recommend that you go to one of PMG’s after hours offices, or to one of their urgent care centers.

Lab/X-rays
Specialist Care
Referrals from your PCP to specialists are initiated by your PCP. Additional authorizations may also be initiated by your Specialist. A letter is sent to you to confirm the authorization/referral information.

Identification Cards
  • Your insurance card is an important identifier and should be presented to Providers at the time of service.
  • Do check your identification card (as well as each member of your family) to ensure the PCP you have selected is correct and in your chosen proximity of your home and/or work.
  • If necessary, call the Health Plan to make any changes to PCP assignments.
  • Be sure to keep your card updated when you receive new a new one and to provide a copy of the new card information to your health care providers whenever possible.
PCP Assignments
Once you have selected or changed a PCP assignment to a new doctor, we encourage you to make an initial appointment with this physician. This will provide you the opportunity to meet your doctor, have an initial healthy visit, and to allow the office to collect your insurance information.
